Summary of 2011 PUF User File Changes as of 11/05/2013

PUF Data Dictionary Changes:
Updated data item INSURANCE_STATUS (Primary Payor)
-         Corrected code labels for INSURANCE_STATUS (Primary Payor) in the data dictionary description and the SAS and SPSS scripts.  The data were correct.
The insurance status codes are:
0-Not Insured
1-Private Managed Care
2-Medicaid
3-Medicare
4-Other government
9-Insurance Status Unknown  
 
Updated data item RX_SUMM_SYSTEMIC_SUR_SEQ code values.
-         The descriptions have been corrected for codes 4, 5, and 6. Previously, codes 5 or 6 were not defined, and the definition for code 4 should have been identified as code 5. The information was correct within the SPSS/SAS scripts. 
 
Updated Data Item THIRTYDAY_MORTALITY
-         Removed THIRTYDAY_MORTALITY Code “8 – No surgery was performed.” Code 9 includes patients who did not have resections and also cases for whom it is unknown if a resection was done.
SAS and SPSS Script Change – Use revised scripts which includes all updates 
-         The SAS and SPSS scripts were updated to reflect the revised code values for the following PUF variables:
·        INSURANCE_STATUS (Primary Payor) - The insurance status codes are:  0-Not Insured, 1-Private Managed Care, 2-Medicaid, 3-Medicare, 4-Other government, 9-Insurance Status Unknown.
·        THIRTYDAY_MORTALITY - Removed THIRTYDAY_MORTALITY Code “8 – No surgery was performed.” Code 9 includes cases who did not have resections and also cases for whom it is unknown if a resection was done.
 
-         You will need to change your SAS /SPSS script with these corrections or you can download the revised SAS and SPSS scripts: SAS - http://ncdbpuf.facs.org/?q=node/318, SPSS - http://ncdbpuf.facs.org/?q=node/317.  The revised versions of the SAS and SPSS scripts are included in your new PUF download file.     
 
Survival Rates
As indicated in the “Getting Started” document, misleading survival rates can be calculated if cases diagnosed more recently than 2006 are included in the calculations.  This is a function of the timing of follow-up, registry update, submission to NCDB, and cutting the file for use in the PUFs that result in erroneously high calculated survival rates.  Therefore, all data items used specifically for survival calculations are omitted from the PUF for cases diagnosed in 2007 or later.  The data item “Vital Status” was omitted for cases diagnosed 2007 or later, as intended, but “Last Contact or Death, Months from DX” for those same years was not previously omitted. This was corrected; follow-up is not as complete for cases diagnosed during 2007-2011, so survival may be overestimated for those years.  NCDB reminds PUF investigators only patients diagnosed before 2007 can be examined reliably or meaningfully for survival analysis. 
 
Summary of Updates to PUF Variables:
 
Updated non-primary site surgery codes
Corrected the denominator for the following variables: 
-         Thirty day mortality (THIRTYDAY_MORTALITY)
·        Now limited to cases with Surgical Procedure of the Primary Site = 20-90
-         Days from diagnosis to treatment (DX_RX_STARTED_DAYS)
·        Now excludes cases with no surgery other than regional lymph node removal (which would indicate a diagnostic procedure)
-         Days from diagnosis to first surgical procedure (DX_SURG_STARTED_DAYS)
·        Now excludes cases with no surgery other than regional lymph node removal (which would indicate a diagnostic procedure), and excludes Surgical Procedure of the Primary Site = 98
-         Days from diagnosis to most definitive surgical procedure (DX_DEFSURG_STARTED_DAYS)
·        Now based on cases with Surgical Procedure of the Primary Site = 10-90
-         Readmission within 30 days of surgical discharge (READM_HOSP_30_DAYS)
·        Now based on cases with Surgical Procedure of the Primary Site = 10-90
-         Radiation surgery sequence (RX_SUMM_SURGRAD_SEQ)
·        Now excludes cases with no surgery other than regional lymph node removal (which would indicate a diagnostic procedure), and excludes Surgical Procedure of the Primary Site = 98
-         Systemic surgery sequence (RX_SUMM_SYSTEMIC_SUR_SEQ). 
·        Now excludes cases with no surgery other than regional lymph node removal (which would indicate a diagnostic procedure), and excludes Surgical Procedure of the Primary Site = 98
 
 
Updated PUF data item FACILITY_TYPE_CD (Facility Type)
Corrected classification problem with hospitals to Cancer Program Categories for “Other specified types of cancer programs” and “Academic/Research Program” (FACILITY_TYPE_CD).  Some hospitals were incorrectly assigned to the “Other specified types of cancer programs” category instead of the latter.

Principal Investigator Questions Resolved
 
Question about staging information for Prostate, T2c.
Corrected for Prostate and updated staging rules for Stomach, Soft-Tissue for diagnosis years 2010, 2011.
Question on how sites are categorized and classified for CNS & Skin, Lymphoma.
Update using the “SEER Site Grouping” which categorized these lymphomas into the correct category.
Corrected lymphoma topography codes.  Some lymphomas were distributed with their topography group rather than in the lymphoma files.  That has been corrected.
