It refers to the last date that first course surgery of the primary site was performed for the patient. For example, a breast cancer patient may have been treated with an excisional biopsy, followed by a lumpectomy, followed by a mastectomy. This item identifies the time period between the Date of Initial Diagnosis and the date of the mastectomy. The Surgical Procedure of the Primary Site will record the mastectomy.

This refers to the final surgery of the primary site, cumulative for all procedures, for the cancer by any facility.
